1.) Prophy appointments at your practice MUST BE A MINIMUM OF 45 MINUTES (with the exception of pediatric and assisted hygiene).
2.) A lunch break is required in accordance with New York State Labor Laws.
3.) Hygiene instruments and PPE (gloves, face shields, masks etc.) will be provided by your practice. I will bring my own safety eyewear, scrubs, and lab coat.
4.) All equipment in hygiene operatory is in good working condition.
5.) Payment is based on an hourly rate and is expected at the end of the day (or on a weekly basis if hiring for multiple days) unless other arrangements are made in advance. You agree to pay the agreed upon hourly rate for all hours worked as discussed and outlined on this contract.
6.) If hired for less than 4 hours, a minimum rate will be charged equivalent to 4 working hours.
7.) Scheduling is subject to change. In the rare event that I can no longer accommodate requested days or hours, I will contact your office as soon as possible.
